The Health Connections Clinic provides a unique, team-based approach when caring for people with complex medical and/or socio-economic needs. Care is provided on a case-by-case need with specialized treatment tailored to the needs of each individual. This could include mental health and substance use issues, homelessness, poverty, complex chronic disease, and alcohol use disorder treatment. The Health Connections Clinic team is comprised of family doctors, nurse practitioners, social workers, registered nurses and administrative staff to best support the individualized needs of each patient.
Who Can Access the Health Connections Clinic? Anyone unable to find a primary care provider, and also has medical and/or non-medical needs. Note: Patients wishing to access these services and already have a Family Doctor/Nurse Practitioner, please contact the Comox Valley Addiction Clinic (self-refer accepted).
To Access Health Connections OAT Clinic:
- Patients can call to book an initial intake appointment with a primary care nurse, or to book a follow-up appointment, at 250-331-8502
- Patients can be referred by a community agency, Island Health professional or other healthcare provider (see physician information section below for more information)
- Note: Patients may drop-in to book an appointment, or if they have any inquiries
