Provide education and support to people living with or at risk of diabetes.
All Diabetes Education Centres have modified our service delivery in order to support physical distancing and safe patient care.
The Diabetes Education Centre provides Pre-Diabetes, Diabetes group and individual education: diet and lifestyle self-management; medication review/consultation, insulin start/adjustment, blood glucose monitoring training and certification, insulin pump training, continuous glucose monitoring training, carbohydrate counting, and foot care.
The clinic provides Individual or followup appointments, classes and group sessions and drop in.
Drop in is only Wed 1:30-3:30pm.
- All referrals will receive a phone call to determine the support needed.
- Clients will receive phone consults and will receive education resources by email.
- MyVirtualVisit, a secure video solution allowing clients to use their personal smartphone, tablet or computer, will be available.
