Community-Led Crisis Response Team (CLCR)
Mental Health - Adult & Senior
Provided by AVI Health and Community Services
The CLCR Team is an alternate approach to responding to someone in crisis that focuses on health, well-being and community support. Utilizing a community-based, client-centered, trauma-informed response, a peer-assisted model of care has been proven to help individuals navigate mental and social care services and reduce engagement with the justice system and emergency services.
How it works:
- 1) A crisis occurs and you call 250-818-2454.
- 2) A screening and assessment service worker completes an assessment of client's needs over the phone/text.
- 3) The CLCR team dispatches which the service worker remains on the phone.
- 4) The team arrives. Client-centered, consent based care, immediate crisis management, resource navigation and offer to follow up.
- 5) 48 hours later follow up and provide any follow up care.
